Role Overview

We are seeking a Social Worker to deliver community-based services from Ennis Primary Care Centre. The role focuses on early intervention, support, and integrated care.

Location: Ennis Primary Care Centre, Station Road, Ennis

Key Responsibilities

• Provide community-based social work supports
• Work closely with primary care teams
• Deliver client-centred interventions
• Support vulnerable individuals and families
• Participate in multidisciplinary case discussions

Requirements

• Degree in Social Work (CORU registered)
• Experience in primary care or community services desirable
• Ability to manage a varied caseload
• Strong problem-solving and communication skills
• Full driving licence and access to a car is essential

Hours

• Full-time role
• Reduced hours may be available
