Key Responsibilities

• Support patients and families with emotional and social needs
• Facilitate discharge planning and community referrals
• Collaborate with healthcare professionals
• Provide advocacy and safeguarding support
• Maintain professional documentation standards

Requirements

• Recognised Social Work qualification
• CORU registration essential
• Experience in healthcare settings advantageous
• Strong communication and teamwork skills
• Must have own transport and full driving licence

• Full-time position
• Reduced hours considered
