Care Team Leader

Liverpool

Starting salary: £25,000 - £28,000

Working 40 hours per week over 3.5 days

I am working with a popular care provider who are dedicated to providing exceptional care to individuals in the Liverpool  community.  They pride themselves on delivering  the highest standards of care and are looking for a skilled and passionate Care Team Leader to join the team.

I am seeking a motivated and experienced Care Team Leader who is proficient in managing electronic care plans to oversee the daily operations of our care team. The successful candidate will have a strong understanding of electronic care management systems and be able to effectively support both staff and clients to ensure quality care delivery.

Key Responsibilities:

• Lead and supervise a team of care staff to deliver high-quality care to our clients.
• Develop, maintain, and regularly update electronic care plans using the company’s digital system.
• Ensure that care plans meet the specific needs and preferences of each client while adhering to company policies and regulatory standards.
• Monitor and evaluate the care provided to ensure the highest level of service and client satisfaction.
• Liaise with other healthcare professionals, families, and external agencies to ensure holistic care is provided.
• Conduct regular audits of care plans and contribute to continuous improvements in care delivery.
• Deliver person centred care to residents

Requirements:

• Proven experience in a care team leadership role with a focus on electronic care planning systems.
• Strong knowledge and experience using electronic care planning software
• In-depth understanding of care standards, regulatory requirements, and best practices.
• Compassionate, dedicated, and able to motivate and inspire a team.
• NVQ Level 3
• Experience in Dementia care, residential and elderly care
